Use of Google Forms

ARISA Lab uses Google Forms for surveys including surveys that require the input of Personally Identifiable Information.  Google Forms and GSuite Enterprise utilities meet or exceed HIPPA and European Union GDPR standards.  Google Forms will be accessed using two-factor authentication protocols from private secure servers that are accessed using Transport Layer Security Protocols to ensure security and avoid “man-in-the-middle” attacks.  Once the forms are filled out the data will only be stored in secured servers using AES-256 encryption.  The data will be stored locally and in an encrypted backup located in Google Cloud Platform’s North Virginia complex.

Privacy

All personal information collected are in the form of voluntary questionnaires.  Email addresses are collected for matched pre/post comparisons only. Email addresses will not be shared with 3rd parties or used for marketing purposes.  Zip codes are used for demographics data only and will not be used in conjunction with personally identifiable information. 

Updates to this Policy

We reserve the right to update this policy at any time to take account of new data protection legislation and to reflect our current business practices. If we do change the policy, the new policy will be posted here, along with its revision date. We encourage you to check this policy periodically and before each instance in which you provide personally identifiable information to us.
